nrinno @splitpea3 Hi! It's 100% cotton with no stretch, so I would say it is definitely true to size Medium. I also wear sz Medium and 19-19.5" WITH stretchy fabric like spandex or elastane is the norm. You'd want an extra inch to allow for ease of movement since it's all cotton.
