Antique Handwritten Sermon Notes Written On Long Distance Medical Questionnaire

Vintage handwritten sermon notes / devotional thoughts on the back of long-distance medical consultation by mail forms by Dr. Greene. It appears that Dr. Greene had an office at the New York address listed between 1887 and 1901. The questionnaire consists of a series of questions that the patient would answer and then a long-distance diagnosis would be done by Dr. Greene. From other things found when researching the medical form it appears that Dr. Greene was also involved in quack medicine tonics. A unique and historical collector's piece. There are some tears along the edge of the pages and also along the creases where the pages have been folded. See pictures for further details on condition.
