@mydetailman measure from the seam at the top of the sleeve head where it joins the shoulder and finish the measurement at the very end of the cuff. That's the sleeve measurement.
For the shoulder: across the back of the coat, go from the seam where the sleeve head meets the shoulder, same as before, and measure across to the same seam on the other shoulder. This is sometimes called the point-to-point measurement, in case someone else asks.
