Huge vented locket/ trinket box necklace

I'm told it's from the Middle Ages when the wealthy wore vented lockets with oil soaked fabrics, to ward of the smell from the plague. It's obviously hand-custom made and it is not stamped. The back hinge is a little out of alignment.
