Does it run small or narrow?

@dawnamic normal. The size on the shoe indicates 6B (medium width). They are leather and will also form to your foot. I am usually a size 6.5 and thought they would fit but they did not. There is a small crease on the front because the leather is soft and as soon as you step the crease forms which is why the company does not take returns and I could not return them.
