The skirt is a 3 hoop cage skirt with 4 layers of hand gathered tulle and mesh. If you look even closer there are little ruffle details along each layer. The costume also includes a pair of tights, the polka dot gloves and the little hair bow head piece, as well as the harness strap detail.
