Do these fit fairly true to size or do they run a little more narrow?

@brynlebaron the base fits true to size and can be additionally adjusted with the straps. I have wider feet and they are very comfortable. The heel might be a tad narrow visually - I'm not sure how wide your heel is, but it's just typical for a stiletto mule and doesn't disrupt comfort of the shoe. That is nitpicking.
